I have over 20 years of experience working in the field. I am a graduate from Eastern New Mexico University and received my degree in Social Work. I have worked with persons of all ages and diversity and continue to enjoy working with so many amazing individuals. I have knowledge working with SSA, elder services, child services, social services, Medicare, Medicaid, disability, veterans and many other programs in NM. I enjoy being a part of Visions Case Management and providing services to so many wonderful individuals who receive DDW services across the state.
I am native to northern New Mexico and cannot see myself living anywhere else as NM has so many wonderful things to do and experience. I come from a large family and enjoy so many of New Mexico’s wonderful traditions, foods and culture. New Mexico’s diversity and beauty is what I enjoy the most. I love being outdoors and spending time with my family. In my spare time, I love hiking, fishing and taking pictures of our beautiful state. I enjoy meeting new people and being introduced to different cultures and foods, so I take the opportunity to try new things.

I have been working in social services for 12 years. I joined Visions in 2025 and am happy to be part of this team! I have a BA in Art Therapy and graduated from The Evergreen State College in Olympia, WA. I have worked with both youth and adult populations in education and therapeutic settings throughout my career. With Visions I am the DD Waiver Regional Lead serving the northeast region of the state. Originally from the northern Illinois area, I have lived in New Mexico for 7 years. I lived in Jemez Springs and now Albuquerque, and have a deep fondness for this state and the people living here. I enjoy being outdoors and in the mountains hiking, climbing... Moving is important for me. I teach yoga, and have been practicing close to 20 years. I enjoy playing guitar and making music with others. Tea, biking, dancing, reading, and a good belly laugh are simple things I enjoy.

I joined the Visions team in May 2018 but have been in the field for about 7 years in different roles prior to this position. Most of my background is in the social services sector. I’ve worked in different roles in the DD waiver and did case management for troubled youth for some time. I was born in California and moved to Santa Fe New Mexico with my family when I was 3, I have been here since then. I have served different regions of New Mexico along my years of experience; in my previous job, half my caseload resided in Espanola and in Rio Arriba county. In my current position with Visions, I serve in Santa Fe and Albuquerque. I’m extremely passionate about my job and enjoy what I do very much! I have two boys that keep me extremely busy when I’m not working. All the things I enjoy doing outside of work revolve around them. Some of my favorite things to do with my children include road trips, camping, and being involved in their school events. During my free and alone time, I enjoy hiking and love a spa day when I can get away with one!

I have been with Visions Case Management for almost 5 months. Before I became a Case Manager, I worked for another agency in the DD waiver field. I have an abundance of background experience in the following roles/departments: Direct Care Staff, Quality Assurance, Human Rights Committee Coordinator, House Manager, CCSI Manager with 30 Clients and 6 Staff.
I wanted to become a case manager and utilize my skills that I have learned throughout my years of experience in the field. After a year of asking many questions and getting answers from other Case Managers, I finally decided to fix my resume and apply for a job with a case management agency. I am so grateful to Visions for giving me the opportunity to become a Case Manager and utilize those skills. I enjoy the outdoors and going to our family Cabin. I enjoy hunting, fishing, and making memories with my husband and grandkids. This job has allowed me to do all the wonderful things that I enjoy; I have met so many new families and have had the opportunity to help them with the things they need to thrive. Visions is a wonderful place to work, as the other Case Managers, admin and Supervisors are so helpful and kind. I work hard and try to do the best I can, as I am still learning something new every day.

I have been working as an independent contractor in the Developmental Disabilities Waiver program for more than three years. I joined the Visions Team as a Case Manager in 2022. I hold a Bachelor of Arts in Multi-Disciplinary studies from the University of Texas at El Paso with areas of concentration in Human Behaviors and Interactions as well as Spanish Literature. This degree granted me flexibility in my interpersonal relations skills. I have been providing services in every region of New Mexico where I am needed but I mainly oversee the Southeastern region serving the Lea, Chaves, and Lincoln counties of New Mexico. I have more than 20 years of experience in the health care industry working with individuals with special needs in the context of a medical office. I administratively led a medical practice that worked with patients with down syndrome and autism. I am a native Spanish speaker, and this allows me to communicate effectively with individuals that may not be fluent in the English language. It is always my goal to go above and beyond in support of my clients. I always advocate for their best interests, and I like taking the time to personally get to know each client and their family, to learn about their goals, and their aspirations. I pride myself in providing excellent customer care to the persons entrusted to me under the DD Waiver program. When I am not working, I love spending time with my beautiful family and our 3 dogs. I also enjoy traveling and spending time with friends watching sunsets, playing outdoor sports, cooking up barbeques, bowling, and playing dominoes.
I look forward to the privilege of serving you!

I have been with Visions since 2023 and in the field working specifically with people with intellectual disabilities since 2014. I have worked as a direct care staff in supported living and ICF homes. I was promoted to Program Manager in Supported Living, then QIDP Asst. in ICF where I managed homes and the active therapy provided to residents. I then moved to CA in 2018 and worked in Supported Living managing homes across the East Bay. I discovered the role of a case manager in CA and worked as one for a few years during Covid. I felt I found my place in the field as a case manager. In 2023, I moved back to Albuquerque and started working with Visions Case Management. It's been a great agency to represent and the support is amazing. I have learned a great amount in this position and plan to stay in the field.

My name is Tracy Reeves and I joined Visions Case Management in 2024. I have a son who is autistic, so I love to support parents, children and adults to get the services they need to live life to the fullest. Before doing case management, I was living in Peru as a missionary and one of my main duties was to help individuals find proper medical care. My son grew up in Peru, so I became familiar with the medical community as I sought resources for him. When I am not working, you will find me at the gym. I enjoy being active and am working toward the goal of completing the Camino de Santiago in Spain.

I have worked in the DDW field since 2004, as a Behavior Support Consultant, Case Manager and now as a Guardian/Court Visitor. I am currently managing both the Guardianship and Court Visitor programs, but my passion is always to work directly with the individuals we serve. I was born and raised in Northern New Mexico and after being away for ten years for school, decided it was time to come back to my roots. I am honored to provide quality services to those in need and believe we have one of the strongest and most compassionate teams of people working at Visions.

Hello, My name is Roxana Lazaro. I graduated from New Mexico Highlands University in 2021 with a Bachelor of Arts in Psychology (Minor is Sociology). I have various years of office management experience in a mental health setting, and also have business administration experience as a mid-level manager. I have been in New Mexico for approximately eight years; I have previously traveled to/lived in: Socorro, Las Vegas, and other points within this state. I arrived in Albuquerque at the precipice of the 2020 COVID-19 global pandemic. I enjoy listening to music, bodily exercise/movement, the arts, and the sciences. I am bilingual in Spanish/English. My work with the New Mexico Disability Waivers has led me to a better understanding of the coordination efforts present throughout the state as it continues to mobilize to protect those that may not be able to speak for themselves as a result of an intellectual or developmental disability.

I’m so excited to be working for Visions Case Management and Consulting. I grew up in western New York in a very small town where my family is from. My great grandmother even taught in a one room schoolhouse there. I received my masters degree from Ithaca College in Occupational Therapy in New York. I have lived all along the east and Gulf Coast. I landed in New Mexico almost four years ago and I just love it here. I am excited to support programs that allow people with disabilities to participate in life and contribute towards their community. As an occupational therapist I believe that all people should have access to services that allow them to live life to the fullest. I love to watch others grow to their best potential. When I’m not working, I’m helping my partner with her three kids—one of whom has special needs—as well as tinkering around the house. I also enjoy looking for ways to enrich my community and spend time outdoors.

Hi, I’m Elena and I am very proud to be part of Visions as a Consultant for the Mi Via Waiver. Carolane McNees, the founder of Visions Case Management, was my daughter’s first case manager and I had the pleasure of being with Visions when it first began. I was born and raised in Santa Fe, got married and moved to San Bernardino, California where my husband was stationed in the Armed Forces, where I gave birth to my son who has married and given me 6 beautiful grandchildren. Later, I moved to Salt Lake City, Utah where I gave birth to my daughter who was born with multiple disabilities. We decided to move back to Santa Fe to be close to family and have lived here for the past 37 years. My experience comes from being a mom and wearing many the many hats that need to be worn while being the strongest advocate possible, going through the many challenges that only those who walk or have walked in the shoes can truly understand what it’s like day to day. The tremendous Love, Joy and Compassion learned from taking care of a person with special needs is remarkably unique and so special. I have been a caregiver, staff, transporter, and trainer for many individuals with special needs for 39 years. After my daughter’s passing 9 years ago I began my new-found journey in honor of her memory. I became a Coordinator for New Pathways for 2 years, then moved over to Santa Lucia where I enjoyed working for 7 years until I was offered a position to be a Consultant with UNM and worked for 1 ½ years. I have been with Visions since May of 2017, and I am honored to be part of a wonderful team at Visions and love working with all of the families who have welcomed me into their lives. When I am not working I spend a great deal of time at the grandkids’ basketball games, fishing, gardening, and going for nice scenic drives.

I have been on the Visions team as a Mi Via Waiver Consultant since 2023. This was also my first experience working with Mi Via. I was a DD Waiver Case Manager in Indiana for almost 7 years and a Mental Health Life Skills Coach for 3 years after that. I am an advocate for all. I am originally from Fort Wayne, IN and moved to Albuquerque in November of 2019. I currently serve the counties of Bernalillo, Valencia, San Juan, and Catron. When I am not hard at work, I enjoy traveling, hiking, visiting with friends and family across the country, and going to as many concerts as possible.

Hi, I’m Gladys and I have been a Mi Via Consultant with Visions since September 2020. I’m a Licensed Master Social Worker in the state of NM and I live in Las Cruces with my wonderful husband and our 2-year-old son. I currently serve individuals and their families in the counties of Dona Ana, Luna, Otero, and Grant. Prior to working at Visions, I worked as a middle school social worker and before that, I worked as a care coordinator for an MCO. In my free time, I enjoy spending time with my family. My son keeps me busy, and we are always doing something! Whether that’s going to the park, the local splash pads, or the library. We also enjoy traveling, exercising together, and going to church.

In November of 2017, I had the pleasure of joining the Visions Case Management team as a Support Broker. My professional and personal experience goes back many years. Since 2001, I have had the honor of working with many of New Mexico's best nonprofit organizations and with various vulnerable populations. I have enjoyed serving individuals who have developmental disabilities, severe mental illness, struggles with homelessness, and children with profound medical needs. Along with my professional experience, I have 19 years of personal experience given to me by my son who had profound developmental disabilities and was medically fragile. My son passed away in 2008 at the age of 19 years old. The experience my son provided me has given me the ability to view the world through eyes that choose to see love and a heart that feels compassion. After my son's passing, I pursued my education and obtained a bachelor’s degree in criminal justice. I am a native New Mexican and live in Albuquerque where I enjoy being active in my church community, spending time with my family, enjoying nature, reading, and most recently trying my hands at arts and crafts. I currently serve members in Central, Eastern and Southern New Mexico and the Albuquerque area. I look forward to meeting you!

I moved to New Mexico from New York in 2008, and while I remain a New Yorker at heart, I have truly embraced the beauty and culture of New Mexico. I have a degree in psychology and feel passionate about supporting individuals with behavioral health diagnoses, developmental disabilities and medically fragile conditions. Outside of work, I enjoy being outdoors, walking my dogs, spending time with husband, traveling and painting.

I began my healthcare career as a Licensed Nursing Assistant in 2012, gaining extensive hands-on experience in direct patient care across diverse settings, including hospitals, rehabilitation centers, hospice care, mental health, labor and delivery, oncology, and as a Respite Provider. I’ve also worked as a health coach alongside primary care providers and supported individuals experiencing suicidal ideation, serious mental illness (SMI), and intellectual disabilities. I remain an active Licensed Nursing Assistant and continue to provide direct patient care, which allows me to stay closely connected to the individuals and communities I serve.
My background also includes care coordination, where I developed a strong understanding of the challenges faced by our most vulnerable populations and worked to connect them with the care and resources they need most.
Throughout my career, I have held various administrative and leadership roles that strengthened my communication, team management, and problem-solving skills. These experiences shaped my ability to lead with empathy, maintain operational excellence, and foster collaboration across departments.
I am passionate about leveraging technology to enhance processes, support data-driven decision-making, and elevate the quality of care coordination services we provide. Whether managing systems, guiding teams, or developing innovative solutions, my goal is to create meaningful and lasting impact for both clients and staff.
Originally I'm from Guadalajara, Mexico, I relocated from Arizona to New Mexico in February 2025 with my wife of 16 years and our two children. Outside of work, I find balance and inspiration in nature whether I’m creating art, gardening, helping others, or caring for our farm animals.
